Step 1: Provide Business Information

For the kitchen contract, you will need a $1 million liability insurance policy naming Commercial Kitchen Factories Austin as an "additional insured."

Step 3: Upload A Valid Identification

Please upload an image of your valid state identification. It could be a State Identification Card, A driver's License, or A Passport 

Step 4: Pay Fee

Please note: all payments are non-refundable.

Step 5: Submit Form & Pick Up Permit

We'll mail the permit to your address above or you may pick up the permit in-person at our north or south kitchen (by appointment only).
