Core Yoga Registration & Agreement Form
As a part of the conditions of joining any of my classes there is a requirement for you to complete the relevant forms. It is important that I can gather basic information regarding yourself, including your contact details, your communication preferences, some basic health data and I like to hold an emergency contact for all my students.

All clients are asked complete this general information form, together with any specific form relating to their chosen class. Please note I will only text/email/WhatsApp you, with your permission and it will only be in relation to your participation in Core Yoga.

You can be confident that all data is stored securely, and that Core Yoga complies with the General Data Protection Regulations (GDPR). Whether you supply the data digitally, by email or in hard copy, (paper). Your information will not be passed or sold onto any third party or used for any other purposes it was collected for.

If Core Yoga uses a company to provide services such as the website, an app or other such tools, you may rest assured that they too will be fully compliant with GDPR and that your data will not be compromised in anyway. If you have any questions regarding the security of your information, please do not hesitate to ask.

I understand that Core Yoga will only use my data in the stated methods and I have read all the information and I understand that by signing this form I am agreeing to all the terms and conditions.
