    I understand that as a godparent/sponsor I am to live a life in harmony with the nature of this responsibility, meaning:

    • I am a Roman Catholic, who is a registered, participating member.
    • I have been Baptized, Confirmed, and have recieved First Eucharist in the Catholic Church. (CIC 874.3)
    • I am at least sixteen (16) years old. (CIC 874.2)
    • I attend Sunday Mass weekly & Holy Days of Obligation. (CIC 1247)
    • I receive the Eucharist regularly. (CIC 920.1-2)
    • I go to Confession (Penance) regularly. (CIC 989)
    • I am not the parent of the individual I am sponsoring. (CIC 874.5)
    • If married, I am in a valid, Catholic marriage. My marriage was celebrated according to the laws of the Catholic Church, either as a Catholic marriage, or a marriage validated by the Catholic Church. (CCC 874.4)
    • If single, I do not live in cohabitation, nor am I an active homosexual.
    • I seek to the best of my understand of my Catholic faith to be faithful to the teachings of the Church.

    I understand that being a godparent/sponsor is a commitment of time. I will attend catechetical sessions, the celebrations of liturgical rites, and other events to the best of my ability.

    I understand that a godparent/sponsor has a special relationship to the person who is to be baptized and/or confirmed. A godparent/sponsor serves a a model of Christian living for the individual he/she is serving, as that person is renewed and strengthened by the power of the Holy Spirit.

    I understand that being a godparent/sponsor is a lifetime commitment to be, as I am able, a faithful witness to the Catholic way of life to the individual I am called to serve.

  • Please note, you will still need to provide:

    • Sacramental Certificates: Baptism, Confirmation, Eucharist OR a Baptismal Certificate with Notations
    • Matrimony Certificate: if Married, a copy of the church Matrimony Certificate
    • Letter of Good Standing: If from another parish, a letter signed by the Pastor, attesting your good standing.
