I agree that I have requested Linda's Pet Care to take care of my pet(s) whil I am away. She has permission to take my pet(w) to the veterinarian in my absence in the event of an emergency. I agree to pay charges for all services provided by Linda's Pet Care and any additional fees for any veterinary services.
