By submitting this form I verify that my teammates and I are both current eligible members in leagues (may be from separate ACS leagues) sanctioned with the American CueSports Alliance, Canadian Cue Sport Assn. or MPA and that we are not professional players by definition of the ACS. We have read and agree to abide by the rules and regulations for Team Competition and to all other regulations implemented by the American CueSports Alliance. Also by submitting this entry form on behalf of myself and my teammates, I am giving our express permission for the ACS to use our photograph for reporting and promotional purposes. ALL TEAM ENTRIES MUST BE SUBMITTED THROUGH YOUR LEAGUE OPERATOR.
Important! For entries postmarked after March 16, 2020, only Traveler's Checks, Cashier's Checks, Money Orders or Credit Cards will be accepted. No entries will be accepted after March 20, 2020.

Refund requests must be in writing and in the ACS office by TOURNAMENT ENTRY April 29, 2020. All refunds will be charged a $10.00 handling fee. All refunds will be mailed after the tournament.
