We also require confirmation from you if you intend returning next year (or if you are leaving us) so please take a few moments to complete this form.

(Cash and credit cards are not accepted for fees)

Your first installment of R5,000 is payable
before the end of November.

Option 1 - over 10 months

in 10 equal payments from January to October	R5,180

5% Fee reduction for early payment	-R2,840
