This form, once submitted, constitutes a contract between the potential speaker and the California Homeopathic Medical Society (CHMS) if you are confirmed as a speaker for our annual conference on June 5-7, 2020, in Corte Madera, California, USA.

A commercial interest is any entity producing, marketing, re-selling, or distributing health care goods or services consumed by, or used on, patients. This definition exempts non-profit or government organizations; non-health care related companies; liability and health insurance providers; group medical practices; and for-profit hospitals, rehabilitation centers and nursing homes.
Some sessions at the 2020 CHMS conference may be recorded and available exclusively to 2020 CHMS attendees to purchase for reference. If so, 100% of proceeds benefits CHMS.
These recordings will not be made available to non-attendees/non-members.
The terms of this agreement are as follows:
Speaker agrees to:
- Provide one 60- to 120-minute educational session at the CHMS Annual Conference (length dependent on time slot provided and agreed upon by the speaker).
- Submit presentation, handouts and any other outstanding materials per the guidelines below no later than April 1, 2020.
- Be responsible for his/her own travel, lodging and meals.
If you are not uploading one with your application, please email your headshot to Kathleen Scheible at Kathleen@bernalhomeopathy.com. The photo should be a minimum 300 x 300 pixels, in .png or .jpeg format, square preferred.
Please submit all handouts and documents that you would like to be made available to attendees prior to the event in PDF format. This includes slide handouts, supporting documents, case studies, reading lists, etc. Handouts will be made available online prior to the conference for attendees to download and view/print. CHMS does not print handouts for attendees.
Presentation Please submit an electronic copy of your presentation in PowerPoint format. If you use Keynote, please go to File > Export and select the .ppt option before submitting. If you would like the full PowerPoint presentation to be included in the handouts, please let us know when you send the file, and we will ensure it is made available to attendees. THIS IS REQUIRED.
Large Files: If any files are too large to send via email (20MB is usually the limit for our servers), please send a download link(s) to Kathleen Scheible at firstname.lastname@example.org using a filesharing service such as Dropbox or OneDrive.
Advanced Content: If your presentation includes videos or non-static images, or if you plan on displaying any documents, these must be embedded into the PowerPoint presentation itself. If you are unable to embed them, please send this content to CHMS well in advance of the conference, to allow us time to coordinate with AV technicians to ensure that your presentation displays properly.
