In consideration of such permission, I do hereby agree to release, discharge, and hold harmless Gulf To Lake Church and their staff, its officers, agents, and employees of and from all causes, liabilities, damages, claims, or demands whatsoever on account of any injury or accident involving the above registered minor arising out of the minor’s participation at Gulf To Lake Church competition and/or activities held in connection with the Gulf To Lake Church Sports & Recreation Ministry. Additionally, I authorize Gulf to Lake Church to photograph my child in promotion of the church’s sports ministry program. I have read and understand and agree to the guidelines as stated.     Your space is not held just by registering, you must also attend one evaluation and pay the registration fee to assure your child's spot on a team. Please don't wait, spaces do fill up, and will be taken first-come, first -served by those who complete all 3 steps -- register, evaluate, and pay. All these details will be sent in the registration confirmation email. **NOTE** If you do not get a registration confirmation email, your registration was not received (and you are not registered!) Please contact us or just fill out another form. (A common error is to forget to hit the submit button AFTER you click the PREVIEW ANSWERS button below, where you will verify your information before submitting.)
