AGREEMENT TO FOSTER
PLEASE READ THOROUGHLY to FULLY UNDERSTAND what you are agreeing to.
By entering his/her signature, FOSTER hereby AGREES to the following terms and conditions:
I understand that I will provide foster care for dog(s) from One Upon a Tail Rescue on a volunteer basis and at no charge to the rescue. I understand that this contract applies to ALL current and future dogs I foster for this rescue, until such time that it may be replaced by an updated version of the contract. The rescue reserves the right to request a new or updated contract at any time. I also agree to the following conditions during the foster care period:
- I will accept and consider the dog(s) as a household companion, NOT as an outside or OUTDOOR PET, and will provide the dog(s) with a safe environment, humane treatment, and give proper food, fresh water, shelter and exercise.
- I understand that the prior history of rescue dogs can make them fearful, prone to ESCAPE or running away, and will take all necessary precautions to esure SAFETY.
- I agree to provide a reasonable amount of TIME for the dog TO ADJUST to new surroundings, and make appropriate modifications to our home/lifestyle to help facilitate that transition, such as a separate room/secure area inside the home.
- I agree to WALK the dog on a LEASH & SUPERVISE at all times when OUTSIDE, including walking on a leash inside of a fenced yard at first, until it is determined that the dog will not try to escape.
- i agree to ALWAYS keep the foster dog(s) in a CRATE/KENNEL when I am AWAY, for safety purposes and for training in preparation for their new home.
- I agree to FEED the foster dog(s) INSIDE of their CRATE/KENNEL & PICKUP all FOOD BOWLS to remove this potential source of disagreement between the foster dog and current pets that may be in the home.
- I understand that the dog(s) CANNOT be taken to DOG PARKS and/or take them to any other place off leash outside of my own home.
- I understand that One Upon a Tail will cover VETERINARY EXPENSES for the dog during the foster period, provided that the veterinary care is PRE-APPROVED and performed by LAKESHORE VETERINARY HOSPITAL in Mandeville, LA.
- I will IMMEDIATELY NOTIFY Once Upon a Tail if the dog is LOST or STOLEN, and I will make every reasonable effort to help recover the dog.
- I will NOT GIVE or SELL the dog to another person, relative, or any other individual, or any rescue group, humane association, shelter or pound, or any medical or experimental laboratory or any organization.
- i understand that all decisions regarding a potential adopter or meet with the dog I am fostering, must be made by Once Upon a Tail, after receipt and APPROVAL of the required ADDOPTION APPLICATION.
- I will NOT alter the dog's appearance in any way shape or form (e.g. cropping, docking) except for routine grooming.
- I understand that Dogs in foster care need to be dropped off at weekend ADOPTION EVENTS and agree to make arrangements to DROP OFF and PICK UP my foster dogs if not Adopted that day.
- I agree to be available for ADOPTION MEETS that have been scheduled with a potential adopter. These meets will be pre-arranged to make sure they meet with my schedule, before the rescue makes a committment to the approved adopter.
- I agree to TRANSPORT the dog to and from VET APPOINTMENTS and to timely make these appointment as requested. These appointments can be arranged to fit your schedule and are at no cost to you.
- I agree to ADMINISTER veterinarian provided MEDICATIONS, such as monthly heart worm prevention WHEN DUE, and pick up at the weekly adoption event or at the veterinarians office if needed.
- I agree to PROVIDE PHOTOS and/or video clips on a regular basis and when requested, to assist the rescue in finding the dog(s) their forever homes.
- I will IMMEDIATELY NOTIFY Once Upon a Tail should the pet become SICK or INJURED.
- I will IMMEDIATELY NOTIFY Once Upon a Tail of any CHANGE in CIRCUMSTANCES that may prevent me from providing foster care during the time period agreed upon.
- I will IMMEDIATELY RETURN the dog in good condition to Once Upon a Tail, if at any point and for any reason, Once Upon a Tail requests that I surrender the dog.
- I understand that the dog has been examined, spayed/neutered and vaccinated if age appropriate by a LICENSED VETERINARIAN or it will be PROMPTLY SCHEDULED, and that the dog's health is accurately represented to the best knowledge of Once Upon a Tail.
- I agree to foster the dog AT MY OWN RISK, and IDEMNIFY AND RELEASE Once Upon a Tail, it's Officers, Directors, and volunteers, of ANY AND ALL LIABILITY arising from damages to person(s), pet(s) or property caused by or associated with the fostered dog(s).
OPTION TO ADOPT A FOSTERED DOG
i further AGREE to and understand that fostering a dog, does not automatically guarantee that I would be approved to adopt the dog I am fostering, should I decide I would like to do at a later date. Adoption of a fostered dog would be subject to the following additional conditions:
- I understand that the rescue does give a dog's foster the first choice at adopting, but that I must SUBMIT an ADOPTION APPLICATION and go through the SAME APPROVAL PROCESS as anyone else who applies to adopt.
- I understand that I must IMMEDIATELY NOTIFY Once Upon a Tail Rescue of my interest to adopt, as I am no longer eligible to have first choice, once another adopter has been approved.
- I understand that my first choice DOES NOT EXTEND to friends or other family members, although we do encourage fosters to have friends and family apply, if you believe they would have the ability to provide both a safe, loving home and the required veterinary care.
PUPPY & UNVACCINATED SAFETY PRECAUTIONS
I further AGREE to the following statements about KEEPING PUPPIES, UNVACCINATED DOGS OR NEWLY VACCINATED DOGS SAFE, and have READ THESE STATEMENTS CAREFULLY:
- Until your puppy is better protected, have them potty on potty pads, and not placed on the ground, grass or dirt outside. Limit your puppy or unvaccinated dog's exposure to other dogs until he has had his first two sets of vaccinations. Vaccines are not effective immediately, so please allow a week past the vaccination date for protection.
- AVOID places where your puppy or unvaccinated Dog could be exposed to parvovirus from unvaccinated dogs. Dog parks, pet stores, play groups, and other public areas should be avoided until your dog or puppy is fully vaccinated.
- When visiting the vet for wellness check-ups and vaccinations, CARRY your PUPPY in your arms rather than allowing them to walk outside and leave him on your lap while waiting in the lobby. Walking where other dogs have walked and gone to the bathroom will increase your puppy's risk of contracting disease.
- PARVOVIRUS is very difficult to kill and can live in the environment for years. If you suspect your house or yard has been infected, clean with a 1:32 dilution of bleach (1/2 cup bleach in a gallon of water). Regular soaps and disinfectants DO NOT kill parvovirus. Areas that cannot be cleaned with bleach may remain contaminated. Remember, the virus can survive on a variety of objects, including food bowls, shoes, clothes, carpet and floors.
- If your dog or puppy is vomiting, has diarrhea, is not eating or is lethargic, you must contact the rescue or our veterinarian right away. These are all symptoms of parvovirus. Remember, infected dogs may show only one symptom!
PLEASE MAKE SURE YOU UNDERSTAND THE PUPPY IS NOT ALLOWED OUTSIDE IN YOUR YARD OR ON THE GRASS/DIRT WHERE IT CAN CONTRACT DEADLY VIRUSES, UNTIL 1 WEEK AFTER 2nd SET of VACCINES.
