Eligibility Requirements:
1. Graduate of Hortonville High School
2. Future plans of attending a technical, 2 year, or 4 year college in as associates level degree or
above, school must be included in application if it is known
3. College Path, must be included in the application
a. Public health
b. Social work
c. Medical fields ( including public safety )
d. Non profit administration
e. Mental Health Related

4. GPA: No GPA Requirements for scholarship, but it must be included in application
5. Community Service Records must be included, including hours completed, location, and service
completed
6. Not limited to age, race, gender, or culture
Selection Criteria:
Selection of recipients will be completed blindly. The board of directors will not know who is
applying when we complete the reviews. We will look at academic achievement, participation in
activities, volunteer and community service work, and all items noted in the eligibility requirements.
The essay will also be used for selection criteria.
Additional Requirements:
1. Must submit a minimum 1 page essay based on the following question.
a. What do you want or hope to accomplish with your education and career goals regarding mental health, and what have you done so far to start your mission?
2. Answer the following 3 questions
a.How has mental illness impacted you personally?
b.How will you contribute to creating reforms regarding how we treat mental illness and the stigma associated with it?
c.Is there a stigma around mental health? How can we break the stigma?
3. Letters of Recommendations. Must submit 2. Can be from the following:
a. Employer
b. Community Service Organization
c. Teachers or Coaches
d. No Family or Friends
