    As an authorized Medicaid provider, Silver Home Care is REQUIRED to collect time and attendance data via Electronic Visit Verification (EVV). 

    An EVV visit includes calls from the client's authorized phone or punches collected by the HHA Exchange App ONLY. 

    With the exception of visits that take place outside the client's home, this form is to be used as a back up only and not an alternative to EVV.  

    A minimum EVV Compliance rate of 90% is expected of all employees. You will not be held accountable for system errors or other issues that are out of your control. If you are having difficulty using EVV, please call us, we are here to help you.  

    Effective January 1, 2023

    Employees who maintain a compliance rate of 95% or more for the month of January will receive a bonus of 25 cents per hour for all regular hours worked from January 1st to January 27th.  The bonus will be paid on February 3rd.  

  • If you are experiencing difficulty using the HHA Mobile APP, you may enter your time by calling 888-206-5826 and following the prompts from the client's home phone. 

    Be sure to get permission from the client before using their phone.

    Please complete this form within 24 hours of the visit and make sure your documentation is complete including A CLIENT SIGNATURE.

    If ANY of the required elements are missing, your submission will not be approved and a manager will need to validate the details of the visit with the client.  This may delay payment of the visit. 

    Do not submit this form before the end of the shift.

  • All missed punches must be verified by one of the following methods

    1. Client Signature and acknowledgement (Preferred)
    2. Client Voice Verification (only to be used for clients who are physically unable to sign) (See below)
    3. Signature verification and acknowledgement by a third party at the residence (only to be used for clients with congntive deficiencies who are unable to verify) 

    Forms submitted without the required verification will DENIED and verified by a manager.  If we are not able to verify the timesheet it will not be paid.
