Please complete this application so Big Love Rescue may assist you in finding a special, compatible companion to join your family. Please understand that not all families and pets are compatible, and we want to ensure the welfare of the animal by placing pets in the best possible home.

WE RESERVE THE RIGHT TO DECLINE ANY APPLICATION FOR ANY REASON.

The local adoption fee is $350. The adoption fee includes the cost of spaying/neutering, heartworm testing if over 6 months, vaccines and microchip. Multiple dewormings and heartworm and flea prevention for all of our pets. It rarely, if ever, covers all expenses, so therefore additional tax deductible contributions are welcomed

Out of state adoption fee for puppies under 6 months old is $300. It includes, vaccines, microchip, multiple dewormings, and a health certificate from our vet for travel. There is an additional transport fee of $185-225 depending on transport company.

OUT OF STATE adult dog adoption fee is $350. The adoption fee includes the cost of spaying/neutering, heartworm testing if over 6 months, vaccines and microchip and a health certificate from our vet for all of our pets. It rarely, if ever, covers all expenses, so therefore additional tax deductible contributions are welcomed. In addition you will have a transport fee.

YOU MUST BE AT LEAST 21 YEARS OF AGE TO ADOPT.
