Travel Waiver/Parental Permission Form for Hosted Participants

To be used by VOLUNTEERS ONLY. Host families and hosted participants should NOT have access to this form. NOTE: For the 2022-23 program year, Independent travel is not permitted. Please do not submit this form for requests.
This form is ONLY used when the hosted participant is not traveling with the host family and/or in which participant is traveling outside of the USA with their own host family or an organized group.

Please consult the following Travel Guidelines to ensure that the proposed travel is in compliance and that a waiver is required.

NOTE: This form is not to be used for Belo travel waivers. Belo sends the waivers directly to the hosted participant to be signed. Please reach out to belousa@belousa.com for any questions.
