PLEASE READ CAREFULLY

Thank you for your interest in playing for Maryland Dynasty Fastpitch Softball, LLC. Please understand this is NOT a recreational program and the focus is on competing at a high level and getting our players to the next level is our priority. The philosophy of this program is to give youth in the Mid Atlantic area an opportunity to play softball at an advanced level while preparing them to play at the next age group or college level.

By Signing Up Below I/We understand:

- This is a significant time commitment to your coaches, team and teammates.

- This is NOT a recreational program and there is NO guarantee of any playing time. Playing time is earned through practice, playing and attitude.

- Practices and games are mandatory. Missing practices and games will affect playing time. If you miss the practice before a game or a game you will NOT start the first 3 innings of the first game you are back. It is then up to the coach when and if you are entered into that game.

- Softball comes after school functions (ie. band, orchestra, science fair, etc.), major religious functions, and major family functions (vacations, family reunions, etc.). Please keep in mind we support you, but if you miss a practice or game there is consequences to your playing time. Your coaches and teammates are counting on you.

- After May 1st softball comes before any other sport (ie. soccer, basketball, etc.) until the end of the softball season. We are ONLY looking for dedicated players.

- During high school softball season we will only practice on Sundays. Practices are manditory.

- Coaches will hold some practices in the offseason (before May 1st). While these practices are mandatory, If you are playing high school softball every effort should be made to attend in order to avoid falling behind the other players (especially pitchers).

- I/We the parent(s) of a candidate for a position on a Maryland Dynasty Fastpitch Softball team, hereby give my/our daughter approval to her participation in any/all activities during the upcoming season.

- I/We assume all risks and hazards incidental to such participation including transportation to and from activities, and I/We do herby waive, absolve, indemnify and agree to hold harmless the Maryland Dynasty Fastpitch Softball organization, participants and persons transporting my/our child.

- I/We understand Medical insurance will NOT be provided by Maryland Dynasty Fastpitch Softball, LLC.

- I/We will furnish a copy of a certified birth certificate with contract and deposit.

- I/We understand that Maryland Dynasty, it's coaches, players and the Maryland Sportsplex which we practice in can not be held responsiable for anything dealing with COVID-19. I/We understand that we are coming to tryouts of our own free will and understand that state, county and MD Dynasty rules apply and I/We agree to obay these at all times. Masks MUST be worn by everyone at all times coming into the facility during posted times, while in the facility and leaving the facil;ity. Coaches and players may remove their mask while in the batting cages, pitching tunnels or fields indoor. Masks are ONLY to be worn during posted times at the facility or by state and county mandates.

- During tryouts ONLY one parent is allowed inside the facility.

- ONLY coaches and players allowed inside facility for practices, workouts, lessons, camps and clinics. We are a training facility.

PLEASE READ CAREFULLY AND SIGN BELOW TO INDICATE YOUR AGREEMENT.
NOTE: THIS FORM INCLUDES A RELEASE OF LIABILITY.

Since participation in youth sports activities can be dangerous, Maryland Dynasty Fastpitch Softball, LLC requires that all participants (and their adult parent(s) or guardians) assume all risks associated with youth softball by signing this general release.

For and in consideration of my child being permitted to participate in Maryland Dynasty Fastpitch Softball youth softball activities, I hereby voluntarily release, discharge, waive and relinquish any and all claims or actions for damages for personal injury, permanent disability, death, or property damage which I or my child may have, or which may here after accrue to me or my child, as a result of my participation in youth baseball activities during play and while I am at the facility while others play or for any other reason. This release is intended to discharge, in advance, Maryland Dynasty Fastpitch Softball, LLC, it’s officers, employees and agents, and the owners and maintainers of any facility used for softball practice or activities, from any and all liability arising out of or connected in any way with my child’s participation in softball camps/clinic activities, even though that liability may arise out of negligence or carelessness on the part of Maryland Dynasty Fastpitch Softball, LLC its officers, agents or employees, or the owners or maintainers of any facility used by Maryland Dynasty Fastpitch Softball, LLC for softball practice or activities.

I further understand that serious accidents occasionally occur during youth softball activities, and that participants occasionally sustain serious personal injuries, death or property damage as a consequence thereof. Knowing the risks, I have voluntarily applied for my child to participate in the activity and thereby agree to assume those risks to release and hold harmless Maryland Dynasty Fastpitch Softball, LLC its officers, employees or agents, or the owners or maintainers of any facility used by the Maryland Dynasty Fastpitch Softball, LLC for softball practice or activities, who (through negligence or carelessness) might otherwise be liable to me or to my child (or my heirs or assigns) for damages.

I further understand and agree that this release, discharge, waiver, and assumption of risk is to be binding on my and my child’s heirs, executors, administrators, and assigns.

I further agree to indemnify and to hold harmless Maryland Dynasty Fastpitch Softball, LLC, its officers, employees and agents, or the owners or maintainers of any facility used by the Maryland Dynasty Fastpitch Softball, LLC for softball practices or activities, for any loss, liability damage, cost or expense which may incur as a result of any injury or property damage I or my child may sustain while participating in the activity.

I agree to comply with the program’s stated and customary terms and conditions for participation according to the Maryland Dynasty Fastpitch Softball, LLC If I observe any significant change with regards to my child’s readiness for participation in the program, I will remove my child from the program immediately.

I understand that with Covid-19 many things could change and have to be moved, resinded or not fulfilled in a timely manner. Please understand Maryland Dynasty Fastpitch Softball, LLC will do everything in its power to follow any and all restrictions or mandates. Maryland Dynasty Fastpitch Softball, LLC will also do everything it can to fulfill or complete any workouts, practices and games that is hampered by Covid-19.

I also understand that pictures and or videos may be taken of my child during tryouts, practices, games and any activities that take place with Maryland Dynasty Fastpitch Softball, LLC and used on social media platforms, our websites, fliers, etc. for the sole purposes of promoting your player and advertising purposes.

I have read this Informed Consent/General Release, fully understanding its terms, that I give up substantial rights by signing it, and sign it voluntarily.
