Thank you for applying to be a rainbow baby model for Michelle Osborne Photography. Baby should be between 8-16 days old at the time of the session. Please fill out the form honestly and in full. You will receive a response via email if you are chosen. Please make sure you check your spam folder as sometimes emails go there if not from a frequent contact. You will receive a complimentary session and 5 high resolution digital images for your time.

(Additional images and products may be purchased, but you are NOT obligated to do so to take part.)

MODEL RELEASE

By signing below, I (Client) am agreeing to allow Michelle Osborne to share images from my session on social media and for marketing purposes without compensation.

Michelle Osborne Photography will retain the full copyright on all images (including digital files) and reserves the right to display, publish, etc. for any reason. Client is prohibited from further editing of images. Client is also prohibited from using photos in contests without written consent from Michelle Osborne Photography. Client may not, for any reason sell their images to any third party. Client will be granted printing rights in writing for 3 complimentary images and is allowed to download and use digital files for printing purposes through me or their chosen vendor (Print quality can only be guaranteed through photographers's preferred professional labs). Additional images or products may be purchased, but Client is under no obligation to do so.
