E. Attach Proof of Polish Heritage. Use birth certificates, passports, naturalization papers, marriage certificates, ship manifests and other official documents to trace your lineage to your ancestor who was born in Poland. It is recommended that you choose the ancestral line which is easiest to document. One line of descent is sufficient proof of Polish descent.
For example: if your maternal grandmother was born in Poland, submit a copy of your birth certificate and your mother's birth certificate. Birth certificates usually include the name, date and place of birth of an individual as well as information about his/her parents including their names and places of birth. These documents together illustrate your ancestral lineage to the person who was born in Poland. Arrange your documents chronologically and scan them to one file. Save your file as, "TR21-Last name-First name-Heritage". Upload below.
