Pre-Application Self-Evaluation Process

Most dogs, with good care, can live to be 10 to 15 years of age or older. Before you adopt, please ask yourself these questions:
- Please consider how old you will be 15 years from now. Will you still be able to care for a dog?
- How many times do you think you might move in the next 15 years? Are you willing to move your dog, too, and restrict your choice of housing to places where pets are allowed?
- What major changes might happen in your life in the next 15 years? Marriage? Children? Retirement? Travel? Relocation due to work? Are you willing to continue spending time, energy and money to care for your dog when taking on new responsibilities or going through life changes?
- Are the other members of your household as interested as you in adopting and caring for a dog? What will you do if your partner, roommate, spouse, children, grandchildren, or parents can't get along with the dog?
- If you are getting a dog for your children, how old will they be in 15 years? Will you still want this pet after the children have grown up and moved out? Do you realistically expect children under age 10 to care for, bathe, trim nails, exercise, train, and clean up after a dog?
- Have you previously owned a pet that didn't live with you for 10 years or more? If so, what happened to it? What will you do differently with this pet to prevent a similar situation from occurring?
- What if a few weeks after adoption the dog develops health or behavior issues? Are you willing (at your own expense) to bring in people with the expertise to help you manage or reverse the situation? Have you looked into pet health insurance?
- Are you expecting the dog you adopt from us to be perfect and adapt to your life within a week? If so, do you think this is a realistic expectation?
- Are you applying for a dog simply because of its looks or breed? If so, have you read the bio and did it seem to fit well with your preferences in a dog? Can you handle it's needs or are you just looking past that and falling in love with a face, hair color or texture, etc?
- Have you read this book yet? If not, please do so before adopting any dog.
Adoption Fee: $250

We are located in rural southwestern West Virginia. Transport fee for long-distance adopters who are more than 100 miles from Lenore WV: $50.

For long distance adopters, adoption/transport fees must be paid and adoption paperwork signed and returned to us prior to the dog's transport to you.

Completion of this application form does not guarantee that ARTBAR will place one of our pets in your care. Completion of this questionnaire authorizes ARTBAR/Jessica White to verify any information contained herein, including verification of medical records of any present or prior pets treated by the veterinarians listed. Completion of this adoption process is an indication that you have read, understand and agree to abide by our Animal Adoption Agreement and that the information contained within this application is accurate and not misleading in any way. All adoptions are at the sole discretion of ARTBAR/Jessica White.

Please give careful consideration to adopting a dog. Dogs are not toys or short-term commitments. Make sure your lifestyle allows the time, patience, training, and expense this dog will need over the years.

All pets will be spayed/neutered, vaccinated, heartworm tested, and treated for parasites prior to adoption. Microchipping is available at an extra cost. Approved local adopters are expected to travel to southwestern WV (vicinity of Lenore) to pick up their pets.

Adopters in northeastern states, we may be able to work with a local rescue near you to facilitate an adoption and/or return. Per our adoption contract, you are not permitted to surrender any of our dogs to a shelter. You must contact us to return or transfer to an ARTBAR-approved sister rescue near you if the adoption doesn't work out.

WE ALWAYS TAKE OUR DOGS BACK IF IT IS NOT A SUITABLE FIT AND/OR YOU ARE UNWILLING OR UNABLE TO WORK THROUGH ANY ISSUES.

Our contact information is on the adoption paperwork. Vetting history is included in the adoption paperwork. Most of our dogs are strays coming from shelters, so the vetting history typically consists of the vetting we have had done while the dog was in foster with us.

PLEASE READ: The adoption fee of $250 covers fully vetting the dog, which includes: spay (for females), neuter (for males), 4DX test (checks for heartworms and tick-borne diseases), rabies vaccination, distemper combo vaccination, wormer, a dose of flea/tick preventative (which will kill fleas/ticks and provide a month's worth of protection against them) and grooming if needed. If you live outside of WV, it will also cover a health certificate for interstate travel. If you live more than 100 miles from Lenore, WV, there is also a $50 transport fee to help cover our vehicle/gas costs.

You are required to take your dog to the vet within two weeks of adoption for a wellness check and fecal test to make sure all intestinal parasites are gone. You must get your dog licensed according to the state/county you live in. You must also get an ID tag with your phone number on it to put onto the dog's collar at the time of adoption.
