The purpose of this confined space training is to:

Ensure the safety of all Validus Energy employees that may be exposed to the hazards of a Permit Required Confined Space.To comply with all regulations outlined in 29CFR1910.146Inform employees of the requirements of the Validus Energy Confined Spaces Program

Responsibilities

Supervisors shall ensure that all employees have the knowledge and skills to comply with this program.Supervisors shall provide the necessary equipment to comply with this program.Supervisors shall verify that no Validus Employee ever enters a Permit Required Confined Space .Supervisors shall ensure that all Permit-Required Confined Spaces permits are posted.EHS shall conduct an annual review of the program including all entry permits issued during that annual period.EHS shall maintain a database of all reclassification forms.All Validus Employees shall complete the training as assigned.

Validus Energy employees shall not enter any permit-required confined spaces. Note: Supervisors may enter a previously designated permit-required space for inspection if the space has been reclassified as a non -permit space using the permit space reclassification form found on the forms page of Validus.Energy.Only approved Contractors may conduct permit required Confined Space Entry.Contractors entering the PRCS must meet all requirements of 1910.146A rescue team must be on-site for the duration of a Permit Required Confined Space Entry.Completed permits are to be sent to EHS@Validus-energy.com

The definitions of a Confined Space and Permit Required Confined Space are Important to Understand. They are:

Evaluation

An evaluation of potential spaces shall be conducted using the flowchart in Appendix 1 of the program and shown below.Note:Examples of potential permit ‐ required confined spaces are: Storage tanks, frac tanks, flow pits, mud pits, compressor coolers, crank cases, cooling towers, fan shrouds and production equipment)

Marking

All permit‐required confined spaces shall be marked with signs or stickers placed on or near the entry point.

All marked spaces shall be secured.

Reclassification

A permit required space may be reclassified as a non-permit ‐required space if all hazards within the space are eliminated.Each reclassification requires a completed permit space reclassification form found on the forms page of Validus.Energy.The reclassification form must be approved by an Operations Leader and an EHS Representative prior to entry.

Hazardous Atmosphere.

Reclassification shall not be done if there is a hazardous atmosphere..

Termination of reclassification permit.

EHS or the Supervisor cancel the reclassification at the end of the inspection or if conditions in or near the confined space have changed and become potentially hazardous.
