FOSTER HOME TERMS AND AGGREEMENT

Please read carefully. By initialing a paragraph, you have agreed to that term. Omission of information or failure to answer all questions will result in this application being declined.
1) I, hereafter referred to as the "foster home", agree that the aforementioned cat is being fostered by me solely. I agree that I will not sell, give away, or otherwise dispose of said cat to any person or entity, for any reason*

2) I agree to care for this cat/kitten in a humane and responsible manner and to provide shelter, food, water, and proper care. I further agree that this cat shall reside inside my hone and will not be allowed to roam freely outside.*

3) I understand and agree that Safe Haven Animal Shelter makes no representations or warranties (express or implied) about the cat's temperament and is absolved from any liabilities for future damage or injuries caused by said cat*

4) If fostered cat becomes ill while in my care, I will contact Safe Haven Animal Shelter before taking cat to veterinarian. *

5) I will take responsibility for any injury caused to cat while in my care and pay for any veterinarian costs if needed of said injury.*

6) If I have to return the fostered cat, I will contact Safe Haven Animal Shelter at least one week in advance of surrender date to allow time for Safe Haven to find another foster home.*

7) If I want to adopt my foster cat, I agree to pay the adoption fee to cover Safe Haven's expenses in caring for this cat prior to adoption.*

8) I have read all requested information carefully and certify that all statements made by me are true and accurate. I understand that Safe Haven Animal Shelter has the right to confiscate the aforementioned cat during fostering in the event these statements are found to be false.*

9) TO FOSTER A CAT/KITTEN, YOU MUST READ AND SIGN THIS WAIVER/RELEASE FORM: I and family members agree to wear a face mask during a cat/kitten visitation/meeting and/or at a Safe Haven Animal Adoption event. Although precautions are taken, the risk to have contact with individuals who have been exposed to Covid-19 does exist and it is impossible to eliminate the risk that I and/or family member could be exposed to and/or become infected through contact with or close proximity with an individual with this disease.I hereby fully and forever waive, release, and hold harmless, Safe Haven Animal Shelter, including board members, foster homes, and volunteers, from actions, claims or every nature whatsoever, damages and judgements which I may have now or in the future against Safe Haven Animal Shelter for injuries or sickness to myself and family members, known or unknown, and arising out of myself or family member as a visit, volunteer, and/or prospective adopter of Safe Haven Animal Shelter. I, THE UNDERSIGNED, HAVE READ THIS WAIVER AND RELEASE AND UNDERSTAND ALL OF ITS TERMS AND I EXECUTE IT VOLUNTARILY AND WITH FULL KNOWLEDGE OF ITS SIGNIFICANCE.*
