I/We   First Name*   Last Name*   do hereby give Miss Military Star LLC and Military Star Organization R , its assigns, license and legal representatives the irrevocable rights to use pictures or photographs, names, in all form and media in all manners, including composite, for advertising for
publication or any other lawful purposes, and I waive any right to inspect or approve the finished product, including written copy, which may be created in
connection therewith.

LIABILITY RELEASE FORM

This is to certify that (your name)   First Name*   Last Name* being of self participant  or parent/guardian with legal responsibility for this participant, do consent and agree to release as provided above, and for myself, my heirs, assigns and next of kin. I release and agree to indemnify harmless the Miss Military Star LLC and the Miss Military Star Organization from all liabilities incident to my/my childś involvement or participation in these programs as provided above, even if arising from negligence, to the fullest extent permitted by law. I will not hold the Miss Military Star pageant or any of itś representatives responsible if injury or theft occurs before , during, or after the pageant. I understand that I will be solely responsible for the supervision, safety, care and well being of myself/my child during the entire Miss Military Star Pageant.

of kin. I release and agree to indemnify and hold harmless the Military Star Organization from all liability’s incident to my minor child's involvement or participation in these programs as provided above, even if arising from negligence , to the fullest extent permitted by law.

  FINANCIAL RESPONSIBILITY AGREEMENT

I/ we understand (your name)   First Name*   Last Name*understand that if I/my daughter wins/ accepts an appointed Miss Military Star State /nickname title, we are required to attend Nationals. I also understand by accepting said title I am responsible for all required fees/registration fees associated with the National pageant. I/We understand that there is a payment plan associated with these fees and that if I do not make the payments on time there will be a late fee applied. I/We also understand that if my payment is more than 30 days late I/We forfeit the title and will still be responsible for all required fees.
