I authorize all the schools, persons and organizations named in this application to provide any relevant information in their possession or knowledge to the agents of Pest Patrol, for use in deciding whether or not to offer me employment and specifically waive any required written notification. I hereby release Pest Patrol, my former employers and all other persons from any and all claims, demands, or liabilities arising out of or in any way related to such inquiry or disclosure.
I understand that Pest Patrol is committed to maintaining a drug and alcohol free work place. I understand that if I am extended an offer of employment it may be conditioned upon my successfully passing a complete pre-employment physical examination. I consent to the release of any or all medical information as may be deemed necessary to judge my capability to do the work for which I am applying. Accordingly, I may be subject to a pre-employment blood test, urinalysis or other drug/alcohol screening. I further understand that if employed, I may be subject to such a drug and alcohol screening if Pest Patrol has reasonable suspicion to believe that I am under the influence of a drug or alcohol. My consent to submit to such a test is required as a condition of employment and my refusal to consent shall result in a refusal to hire, or if already employed, termination.
I authorize Pest Patrol to obtain consumer reports from consumer reporting agencies for use in deciding whether or not to offer me employment, to include a background investigation. I understand that such reports may include information concerning my credit worthiness, credit standing, credit capacity, character, general reputation, personal characteristics, or mode of living. I understand that if I am denied employment based upon information obtained in any credit report, I will be provided with the name, address, and telephone number of the consumer reporting agency, a copy of the report, and an explanation of my rights concerning it.
I understand driving is a requirement of the job for which I am applying, and that my employment and/or continued employment is contingent on possessing a valid driver's license for the state in which I reside, and that I have automobile liability insurance in an amount equal to the minimum required by the state I reside.
If hired, I understand and agree that Pest Patrol, to the extent permitted by federal, state, and local law, may exercise its right, without prior warning or notice, to conduct investigations of property (including, but not limited to: files, lockers, desks, vehicles, and computers), and in certain circumstances, my personal property.
At-Will Employer: I understand and agree that the employment for which I am making application is at-will and such employment may be terminated at any time with or without cause, without prior notice by either myself or Pest Patrol. There will be no agreement, express or implied, between Pest Patrol and me for any specific period of employment, nor for continuing or long term employment, unless made in writing, signed by an authorized representative of Pest Patrol.
I understand and agree that as a condition of employment and to the extent permitted by federal, state and local law, I may be required to sign a confidentiality agreement, non-compete agreement, restrictive covenant, and/or conflict of interest statements, as well as an agreement to arbritrate.
If hired by this company, I understand that I will be required to provide geniuine documentation establishing my identity and eligibility to be legally employed in the United States and I also understand that Pest Patrol employs only individuals who are legally eligible to work in the United States.
I certify that all the information on this application, my resume, or any supporting documents I may present during any interview is and will be complete and accurate to the best of my knowledge. I understand that any falsification, misrepresentation or omission of any information may result in disqualification from consideration for employment, or, if employed, disciplinary action, up to and including immediate dismissal, regardless of the time elapsed before discovery.

ONLY THE PRESIDENT OF THE ORGANIZATION HAS THE AUTHORITY TO ENTER INTO AN AGREEMENT OF EMPLOYMENT FOR ANY SPECIFIED PERIOD AND SUCH AGREEMENT MUST BE IN WRITING, SIGNED BY THE PRESIDENT AND THE EMPLOYEE.
IF EMPLOYED, I UNDERSTAND THAT I HAVE BEEN HIRED AT THE WILL OF THE EMPLOYER AND MY EMPLOYMENT MAY BE TERMINATED AT ANY TIME, WITH OR WITHOUT REASON AND WITH OR WITHOUT NOTICE.
