Please dress your children in comfortable clothing so they can move freely. T-shirt is included in the tuition cost. Early Bird Special: $150 (or split into two payments of $80). Apply sunscreen/bugspray if applicable and bring sports/refillable water bottle. Sneakers must be worn to participate (no open toed shoes allowed).

Please note a 3% additional charge is added for credit card payments. You may also pay by check made out to Dominique Paxton, due the first day of Club. ALL children must be registered regardless of payment method.

My child's participation in the selected program is voluntary. I understand that the selected activities may involve accidental injury and hereby voluntarily assume such risks. Knowing these risks, I want my child to participate in this activity. I (on behalf of my child) hereby assume the risks and hereby waive, release, and discharge Elev8 Run Club, its officers, employees, activity instructors/coaches, and assistants, and all officers and employees of the school or community center sites where said activity will take place, for any and all claims for damages for personal injuries, or claims for damages to property, which my child or my child's heirs, assigns, executors, or administrators may have or which may accrue to my child's participation in this activity.

This permission is valid for the Spring 2027.

I have read the above and understand important legal rights are being waived.             

I consent Elev8 Run Club to use of any photographs or video recordings that are taken of my child while participating in the activity for use in brochures and program materials that are distributed both as printed document and on the internet. No payment will be made for use of these photographs and/or videos. Your child's name would never be used in connection with these images.
