My child and I have reviewed the Heritage Elementary School Student/Parent Handbook and the behavioral expectations for the 2026-2027 school year. This document can be found on the school’s website. I understand that the handbook contains information that my child and I may need during the school year and that all students will be held accountable for their behavior and will be subject to the disciplinary consequences outlined in the handbook. All students must have this signed form on file at school.

I, the undersigned parent or guardian, affirm that I have been notified of the key aspects of the Heritage Elementary School policies and procedures including those governing student attendance, conduct and discipline, academic standards and grading, computer use and ethics, and grade level promotion. I have reviewed in detail and understand the policies and procedures in the Heritage Elementary School Student/Parent Handbook and agree to abide by the policies and procedures contained therein.
