    This is a required form for chapter leaders to complete each year. The form provides AHF with simple information about it's leadership, activities, and governing documents that can help us assist chapters and market their events. Someone from each chapter must complete this form every year. We realize form details may not change year-to-year, but still ask that the form be completed, in full, annually none-the-less.

  • Confirm below that you have renewed your insurance for the coming year. Insurance should be renewed annually. If you are signed up with AHF's umbrella Director & Officers Insurance (D&O), we will renew your insurance on your behalf. General Liability insurance should be obtained by the chapter personally and renewed annually. It is not required, but we strongly recommend it. 

  • Please use the spaces below to list out upcoming events including: Member meetings, webinars, virtual conferences, in-person meetings, or in-person conferences. 

  • Please provide us with the costs of your membership dues and sponsorship fees. If you do not charge fees, please simple indicate $0. This helps AHF stay up to date on various chapter fees to better assist you when discussing chapter participation with sponsors and members.
