I hereby apply for water service for the above-referenced property. I understand that this application is subject to approval by AMWC. I agree to use the water service and pay all rates and charges for the water service in accordance with the rates, rules, and regulations of AMWC.

I understand that AMWC will provide me with a quote for the installation fee(s), which must be paid before the meter(s) is/are installed.

A separate meter, or meters, is/are required for each parcel served. A completed, current Will-Serve Letter Request must be on file with AMWC.

This meter must be marked for installation within 60 days of paying fees.  If it is not, AMWC will cancel the application and refund the fees paid. Thereafter, a new application must be submitted and fees must be paid at the rate in effect at the time the new application is submitted.

A connection fee is required for each dwelling on the property. See Policy 1.2.4 Connection Fees for definition of dwelling unit.

 I represent and warrant that I am authorized to sign this application and agree to defend, indemnify, and hold Atascadero Mutual Water Company harmless from and against any and all claims, actions, demands, liabilities, damages, settlements, costs, fees and expenses, including reasonable attorney and expert witness fees and expenses, arising out of or related to any breach or claimed breach of this representation and warranty.

Electronic Signature Agreement

By selecting the "I Accept" button, you are signing this Agreement electronically. You agree your electronic signature is the legal equivalent of your manual/handwritten signature on this Agreement. By selecting "I Accept" using any device, means or action, you consent to the legally binding terms and conditions of this Agreement. You further agree that your signature on this document (hereafter referred to as your "E-Signature") is as valid as if you signed the document in writing. You also agree that no certification authority or other third party verification is necessary to validate your E-Signature, and that the lack of such certification or third party verification will not in any way affect the enforceability of your E-Signature, or any resulting agreement between you and AMWC. In addition, you are confirming that you are the shareholder authorized to enter into this Agreement.
