All new and existing families must complete this one time intake form in order for us to assess your child/s eligibility for our free courses.
Upon submission of your intake form you will receive an email with a link to register for our upcoming courses.
When registering for courses you will only need to provide your child's name, grade and date of birth.
It is important that all information provided in this form is accurate and up to date.
