To make the best use of your workshop meetings with your instructor, you are encouraged to read the following information and sign at the bottom indicating you understand and agree to the information below. This workshop is funded by the North Dakota Department of Health and Human Services, Behavioral Health Division.
About Your Instructor
Your workshop will be instructed by one of the following individuals: Kristin Hoff, PsyD, LP, CMPC or Erin Haugen, PhD, LP, CMPC. Dr. Hoff will teach virtual courses, and Dr. Haugen will teach in-person courses (Grand Forks, ND only).
Drs. Hoff and Haugen are Licensed Psychologists and Certified Mental Performance Consultants ® (CMPC) at Assessment and Therapy Associates of Grand Forks (ATAGF), PLLC. They have doctorates in Clinical Psychology and completed the requirements for licensure as psychologists in North Dakota (Dr. Hoff, #627; Dr. Haugen #402). Individuals who are CMPCs have completed the requirements put forth by the Association for Applied Sport Psychology for this certification. This includes coursework in sport and performance psychology as well as providing sport psychology services under mentorship from another qualified sport psychology professional. They are required to complete continuing education credits in psychology and sport psychology to maintain their licenses and certification. You can reach Dr. Hoff or Dr. Haugen by calling ATAGF at (701) 780-6821.
ATAGF maintains various professional social media profiles to share information about psychology and events that individuals may attend to learn more about these topics. ATAGF has a Social Media Policy that outlines their office policies related to use of social media. This policy is available on the ATAGF website at https://www.grandforkstherapy.com/social-media-policy or by request. You are encouraged to view this policy and discuss any questions you have with your instructor.
Workshop Description
These workshop meetings are designed to assist you with learning coping skills, building resilience, and increasing connection with other student-athletes as it relates to navigating transitions, specifically as you consider retirement from collegiate sport. We will hold 6 workshops over the course of 6 weeks, and each workshop will last 60 to 75 minutes in duration.
Jennifer Haugen, RD, CSSD, LD, will also join us for one meeting to discuss sport nutrition. She is the owner of Northern Edge Nutrition, LLC, and you may reach her via email (jenn@northernedgenutrition.com).
Most RSN workshops will be held live, virtually via a secure version of Zoom. Because we will be sharing course information via email (including email links), by signing this consent form and enrolling in this course, you are consenting to the RSN instructors and Dr. Haugen to correspond with you via email. Please do not use email for anything other than administrative purposes (e.g., informing your instructor you will miss a workshop) and do not use it for emergencies. If your RSN workshop is held in person (Grand Forks, ND, area only), your instructor will communicate the in-person location to you.
RSN is not mental health therapy and is not an intervention to treat mental health difficulties. If you are interested in mental health therapy to treat mental health difficulties, you are welcome to discuss it with your instructor, who will provide you with referrals for this service. If you experience a mental health emergency or crisis, you are encouraged to contact your institution’s Counseling Center or call/text the 988 Suicide and Crisis Lifeline at 988.
If you are interested in individual sport psychology consultations (to improve sport performance), please discuss this with your instructor; they will determine if they or another sport psychology professional can best meet your needs. If your instructor believes that your situation would best be handled through mental health therapy, they will discuss this with you and provide referrals to professionals who can best serve you. Your instructors may be reached by calling (701) 780-6821.
Privacy, Confidentiality, and Security
When people meet with a psychologist, they are entitled to privacy (what information is shared or withheld), confidentiality (the professional’s obligation to respect the client’s privacy), and security of information (regarding access to information).
Your instructor will keep brief notes from workshop meetings that will consist of the date, who was present, topic(s) discussed, and other information helpful in planning future meetings. These notes will be kept on a password-protected, encrypted computer that only your instructor and ATAGF staff (who are trained in confidentiality) can access. These notes will be kept confidential and not shared with others. Because this is a workshop and not mental health treatment, any notes and other materials from the RSN project are not PHI and not subject to HIPAA.
The RSN workshop meetings will include other collegiate student-athletes attending an institution in North Dakota. Although it is difficult to ensure all participants respect confidentiality, we ask that you respect the privacy of any attendee and not share information without everyone’s permission.
To determine effectiveness of the RSN workshop, you will be asked to fill out inventories to measure psychological skills (before the first meeting and after the last meeting) and a program evaluation (after the last meeting) to determine the effectiveness of the program and your instructor’s performance. General findings from these measures and evaluation will be shared with Dr. Haugen (the director of the RSN project) and the funding source identified above. These documents will also be available to you as program participants upon request. Information in these documents will be presented in summary form and will not include any information that can identify you individually.
There are legal and ethical exceptions to confidentiality that require your instructor to act:
- When there is clear and present danger to harm yourself or another person;
- In the case of child abuse or abuse of a vulnerable adult; and
- In the event of a court-order for information.
In these situations, your instructor may involve others to ensure safety, which may include your emergency contact, mental health professionals at your institution, and/or other individuals who can help keep you safe.
Your signature below indicates you have read the above, understand it, have been given the opportunity to get any of your questions answered, and agree to its provisions.
