  • Terms and conditions:

    This agreement outlines the terms and conditions under which we provide services to you (our clients). Please note that where we refer to you below, we are referring to our clients but also any representative authorised to accept these terms on a client’s behalf.

    This agreement helps to ensure there is a shared understanding of:
    - Expectations of services we deliver and how they will be delivered
    - The respective responsibilities and obligations of Allied Therapy Services (us) and our clients (You)
    - How to resolve any problems if they arise

    In the absence of signing this Service Agreement, your use of our services is sufficient to be taken as acceptance in full of the current version of this agreement. It is not necessary for us to get your express agreement or signature to these terms and conditions.

    Unless otherwise agreed in writing, these terms and conditions apply to the exclusion of any inconsistent provisions, which may appear on any form or other documentation issues, including any variations or any additional services provided.

    Scope of Services:
    Services will focus on the goals you would like to achieve and will be provided in your home, the community or other location appropriate for the achievement of your goals. If any safety concerns have been identified for a location, an alternate suitable location will be identified for your session.

    Services provided may be:
    - In person – includes individual therapy, group therapy, carer/educator training
    - Indirect – notes and emails about sessions, case conference meetings, assessment and analysis, report writing, resource preparation, travel (including where applicable km reimbursement)

    Fees and Payment Terms:
    Allied Therapy Services is a private fee paying service. This applies to all in-person and indirect services provided. We accept payments from most major funding bodies, such as NDIS, TAC, My Aged Care, Medicare etc. Please ask if you would like to use funding from another source not listed.

    You acknowledge and agree that we may increase our fees from time to time. We will provide you with at least 28 days written notice of any change. Your continued use of our services will constitute acceptance of the revised service fee.

    Cancellation:
    - Where reasonable notice is provided (more than 2 business days before the scheduled appointment), Allied Therapy Services will re-schedule the appointment with you. No fee will be charged.
    - Where short notice is provided (less than 2 business days before the scheduled appointment) or if you are a ‘no show’, (you fail to attend or be available without notice for a scheduled appointment) we may charge up to 100% of the agreed rate for the missed appointment. We will re-schedule the appointment with you and implement reminder strategies to help remember future appointments if required. 

    You (or your representative) must pay our invoices within 7 days of the issue date using the payment methods set out in the invoice. If there is a gap fee applicable, this will be processed via direct debit.

    If an invoice remains unpaid 7 days after the invoice was due and payable, following a written notice of any late payment, we may pursue the payment of those debts with you directly, and you may be required to pay Allied Therapy Services’ legal and/or other costs in doing so, including (without limitation) administrative processing fees, expenses to a debt collection agency, legal and court fees. We may disclose contact information provided by you to a debt collection agency in a debt collection process.

    If invoices are paid routinely after the due date, we will require a direct debit payment process set up to continue with services.

    Confidentiality:


    We collect your name, contact details and information about your condition and goals to assist us in providing therapy services specific to you. Your information can only be seen by clinicians in this service involved in your support. We only release information about you if you agree or if required by law, such as in a medical emergency.

    We rely on the information you give us to help provide the right service for you. If you decide not to share your information or allow us to contact other health professionals that is your right, however it may affect our ability to provide you with the best possible service. Talk to us if you wish to change or cancel your consent.

    We are committed to protecting the confidentiality of your information, which is also protected by law. We treat your information in the strictest confidence and store it securely. You have a right to request access to your information and to ask for it to be corrected if necessary. Please talk to one of our team if you have any other questions or feedback about what happens to your information while you are working with us, or if you wish to access your information.

    By accepting this Service Agreement you give consent to Allied Therapy Services contacting health professionals or other service professionals for the purpose of obtaining information or providing therapy services.

    Cessation of Services:


    You may cease services at any time by notifying your therapist or phoning (03) 5292 8020. If there are any appointments within 48 hours the ATS cancellation policy will apply. You must pay all outstanding fees up to the date of cessation (whether or not invoiced) and any additional direct out-of-pocket costs or expenses we incur up to the date of cessation as a direct result of such cessation. Providing you give us sufficient notice, we will take reasonable steps to mitigate any such costs or expenses.

    If you have any suggestions, concerns or feedback while working with us please talk to your therapist. If you are still not happy or this is not appropriate you can contact Allied Therapy Services’ Practice Manager on (03) 5292 8020, fill out a feedback form on our website or email admin@alliedtherapyservices.com.au. We will get back to you within 48 hours of you reaching out to us.

    If you are still not satisfied or you don’t want to talk to an ATS team member you can speak to the NDIS Commission on 1800 800 110 or fill out a Complaint Contact Form on their website, www.ndiscommission.gov.au.

    About the support prices:

    Allied Therapy Services charges for time taken to travel to and from home or community-based appointments, in line with the NDIS guidelines. 

    If we provide services to more than one participant within close proximity of each other then we will share the travel time (including return journey where applicable) between the participants seen. Signing this Services Agreement indicates you agree to this. Please note, you will not be charged more than you would otherwise if the therapist was just visiting you.

    In addition to time taken to travel to a client's therapy session, as per the NDIS price guide Allied Therapy Services reserves the right to pass on any non-labour costs associated with provider travel. This contribution will be charged at a per kilometre cost (in line with the NDIS price guide) and will appear as a separate support item on your invoice.

    Prices include non face-to-face activities that also progress you towards your therapy goals, in line with NDIS guidelines.

    Prices and other terms are subject to change.    

    Requests for additional therapy hours are accepted in writing, in lieu of creating a new service agreement.

    • listen to your feedback and resolve problems quickly
    • protect your privacy and confidential information

     

    Your Responsibilities - you agree to:

    • inform us about how you want your supports to be delivered
    • talk to us if you have any concerns about how the supports are being provided or if you would like to make any changes to this Service Agreement
    • provide a minimum of 2 business day’s notice if you cannot make a scheduled appointment
    • provide notice if you would like to end this Service Agreement
    • let Allied Therapy Services know immediately if your plan is suspended or replaced by a new NDIS plan
