Intent to Apply Instructions
1. Complete all sections of this form, the AVMA CVTEA Intent to Apply for Initial Accreditation Form.
2. Once the Intent to Apply has been reviewed and accepted, you will be sent access to the CVTEA Accreditation Portal by Armature to complete an Application Instrument (Report).
3. The application fee for initial accreditation is $3,000 (non-refundable) made payable to the AVMA. The program will be invoiced once the Intent to Apply is approved.
NOTE: An application is considered accepted once the Application Instrument in Armature is approved and site visit dates have been scheduled. Submission of the letter of intent (this form) to apply does not constitute acceptance of an application for initial accreditation.

Staff acceptance of an application or intent to apply for accreditation does not guarantee approval by the CVTEA.

Approval of applications for accreditation is solely at the discretion of the CVTEA

No requests for waivers to the application criteria are accepted.
