Financial Assistance Program Terms and Conditions
I understand the following terms and conditions of the Financial Assistance program:
- Registration for an AlphaBEST program is a separate process from this application, and it is my responsibility to ensure that I have enrolled in the program of my choice.
- This application does not secure a space in an AlphaBEST program and has no effect on any wait-list status.
- AlphaBEST will reach out to me via email once they have reviewed my application.
- In the event that I am approved, AlphaBEST will email me a letter that will detail my co-payment amount as well as the end date for the financial assistance.
- I will be responsible for payment of all weeks enrolled in the program no matter attendance.
- Co-pays are due on Monday of each week and a late payment fee of $15.00 will be applied to my account should payment be received late.
- If I receive an assistance award and I would like to make a change to my enrollment with AlphaBEST during that time I understand that I will need to submit my current income information when I submit my written request. This will allow the financial assistance team to determine if my co-payment amount will change or if my assistance award will end when this change becomes effective.
- Two weeks prior written notice is required to withdraw from the AlphaBEST program.
- Failure to follow the terms of the Financial Assistance Program may result in forfeiture of any assistance award received and our spot in the registered program.
By signing and submitting this I am accepting the terms of the Financial Assistance Program and any Assistance that may be awarded under this program.
