This form is for those interested in being a live model for Yanna, owner of Elluxor Lash. Live model sets are done with a lot of precision and will be photographed and filmed before, during, and after the service.

Yanna only does live model sessions on Mondays-Fridays from 9am to 7pm latest.

How It Works

1. Fill out the form fully and carefully read requirements.

2. If approved, a member of our team will reach out to you within 1-2 business days. We will be sending you a booking link where you can book your model session.

3. On the day of your appointment, please come with your hair styled (straight, curly, or wavy is fine) and bring your makeup with you.

   Do not wear makeup to your appointment. You will be able to do your makeup after the lashes are done on the day of your session.

4. The lash set will be done free of charge.

 Compensation

• Lashes will be done free of charge (originally a $250 value).

• You will have the opportunity to become a reoccuring live model with regular free sets/fills.

 Requirements

Please read carefully! If you are not comfortable with anything below, please do not proceed with the application.

• must be comfortable in front of camera

• must be willing to lay down for up to 5 hours (you will be given a 15-20 minute break halfway, we will also provide snacks/food)

• must sign consent form accepting terms & conditions related to marketing and educational purposes

• you must come to the appointment with no make-up, but bring make-up with you to apply before taking photo and video content

• must have a healthy lash line with a normal to high amount of lashes

• lash set will be chosen for you by Yanna (if you have any suggestions you may bring them up, but cannot be promised)
