    Please fill out this form to request your personal Anvil Zero appliance. How your information is used:

    • Name: Used to provide personalized communication and user tracking. Your name can be different than your Anvil Zero username.
    • Email: Used to send communication from Anvil Zero along with notifications such as password resets.
    • Username: Used to create an appliance that matches your username. This will also be the same username you use to login to Anvil Zero.
    • Password: Your initial password will be randomly generated and sent to your email. This can be reset and changed if desired.
    • Mobile Number: For security, multi-factor authentication is required. Your mobile number will be sent a link to download and install the Duo app for Push notifications. You can alternatively use OTP codes with an app like Authy (recommeded) or Google Authenticator.
