DEADLINE FOR APPLICATION IS FEBRUARY 1 TO BE AWARDED IN JUNE.

REQUIREMENTS FOR ALL FIVE AWARDS: Must bowl in a certified league in Missouri in either the current season or previous season. If ONLY current season, must provide a signed copy of the league sheet showing 21 games. Must be a member in good standing of the Missouri State USBC, and not under suspension by the Missouri State High School Athletic Association. Prior recipients of an award are not eligible to win the same award again. Winners will receive a $1,000 scholarship, sent to SMART.
You will need to fill out this form in its entirety, along with supplying the following information:
- Proof of bowling league in Missouri (prior year or current season)
- Scholastic Transcript
- Scholastic Accomplishments, Community Service, Extra-Curriculars
- Letters of Recommendation (coach and/or school official)
You will be evaluated on each of the scholarships for which you are eligible.
