COLLEGE SCHOLARSHIP APPLICATION FORM

Sponsored by Birmingham Alumnae Chapter of Delta Sigma Theta Sorority, Inc.
Applicants should have the following Credentials:
- Minimum Cumulative GPA of 2.5
- Minimum credit hours 12
- Community Service Involvement
- Communication Skills
- Leadership Skills
- Enrolled at Miles College or Lawson State Community College
· TO APPLY:
- Complete and submit an application form online by 11:59 p.m. CST on March 1, 2026.
- Provide (1) Community Service Letter of Recommendation on Agency Letterhead or online..
- Please copy and send the following Letter of Recommendation Form to the appropriate individuals for submittal: LETTER OF RECOMMENDATION.
- Submit a copy of your college transcript online by March 1, 2026.
- A photograph head shot for display and media purposes (required).
- Complete and submit a Media Release Form.
Notes:
- In order to receive the scholarship award, Final Grades will be verified before scholarship funds can be disbursed.
- Please note that it is the responsibility of the applicant to provide instructions to the community
  service official writing the Letter of Recommendation on their behalf.
Applicants who are awarded scholarships will be notified by April 15, 2026 and will be recognized by the
chapter on May 16, 2026.
