This appointment (or other visits) does not mean I now have a permanent caregiver should I not have one. I know that I can utilize the services of the clinic when available but understand that it is recommended that I seek ongoing care in my community or can request a referral to one if needed.
I can only access care and services when I am at the BREATHE Midwives clinic or have a scheduled follow up appointment during this program in person or virtually.
I understand that this is a free services as part of the BREATH Initiative to provide access to care to individuals that are African, Black and/or Black-Caribbean. I understand that the program will have some limited spots open to individuals who identify as IPOC who may benefit to access. I understand if I do not met the criteria of the program my appointment may be canceled.
