ANY CUTS, ABRASIONS or DISEASES may be turned away. It is politely asked & required for each appointment booking to sign this agreed consent form. If you are showing any signs of illness, please stay home for everyone’s safety & reschedule your appointment or it will be cancelled & no deposit will be refunded unless artist cancels before showing. You AGREE to not hold any accountability or liability of any kind in regards to known or unknown claims against your makeup artist (Nicole Cortes) on or after service.
