Albany Basketball Association Permit Form

Player wishing to play for another Club.
1. This form is to be used for requesting a special clearence to play with another Club in a grade which your Club does not participte in. The permit is specific to the grade and team for which the temporary transfer is requested and is not transferable to any other grade or team;

2. The player requesting the permit must be a financial member of their Club and be a registered player of the Albany Basketball Association.

3. This permit is only valid for one (1) game.
