Blazers Cheer and Dance Evaluation form
This evaluation form is for Blazers Dance and Cheer Elite teams only!

Applicants must have competitive or at least sideline experience at Level 2 or higher in dance, cheer, and tumbling.

Please complete this form and provide a detailed summary of your athlete’s skills and experience in cheer, dance, and tumbling to help our coaches during the evaluation process.
