This agreement (“Agreement”) is entered into between Body By Hound LLC (“Trainer”), and the subscriber (“Client”), collectively referred to as the “Parties.” By signing this agreement, the Client acknowledges and accepts the terms outlined below.
1. Subscription Commitment
• Subscribers are required to commit to a minimum of three (3) consecutive months to access discounted subscription pricing.
• The subscription begins on the date of purchase and renews automatically each month unless canceled according to the cancellation policy.
2. Automatic Renewal Policy
• This subscription will automatically renew each month on the original billing date unless canceled in accordance with the cancellation policy.
• By signing this agreement, the Client acknowledges and consents to the automatic renewal of the subscription.
3. Cancellation Policy
• The Client may cancel their subscription at any time; however, cancellations made before completing the 3-month minimum term will incur a $75 flat cancellation fee.
• Cancellation requests must be submitted through the official online cancellation form, available at: https://form.jotform.com/BodyByHound/SubscriptionCancellation .
• The form must be completed and submitted at least 5 days prior to the next billing cycle.
• Example: If the subscription starts on the 10th of the month, the billing date will always fall on the 10th. To cancel before the next billing cycle, the cancellation form must be submitted by the 5th of the month. If submitted after the 5th, the subscription will renew, and the cancellation will take effect before the following billing cycle.
• Clients may request the link to the cancellation form via email at any time.
4. Billing and Payment
• Subscription fees will be charged automatically on the same date each month as the original purchase date.
• Clients are responsible for ensuring sufficient funds are available for each payment. Failed payments may result in suspension of services.
5. Session Usage and Expiration
• All sessions included in the subscription are 60 minutes in duration.
• Unused sessions remain valid and must be used within 6 months of the date they were acquired.
• Sessions are non-refundable and non-transferable.
6. Amendments
• Body By Hound reserves the right to amend subscription pricing, terms, and conditions with a 30-day written notice. Current subscriptions will not be affected until the next renewal cycle.
7. Governing Law
• This agreement is governed by the laws of the State of New York. Any disputes arising from this agreement shall be resolved in accordance with New York State law.
8. Agreement Acknowledgment
By signing below, the Client acknowledges that they have read and understood the terms of this agreement, agree to the subscription commitment, and accept the cancellation policy, including the $75 flat fee for early termination.
