  • This is the Confidentiality Disclosure Agreement Form (CDA) for CAI's Challenge participants.

    What is an Entrepreneur? Entrepreneurs will form new companies or scale their existing startups.

    What is an Advocate? Advocates are thought leaders, entrepreneurs, investors, subject matter experts, and professionals, who have offered their time to advise/mentor/judge teams, potentially invest in startups, and/or advocate for the CAI challenge-accelerator model.

    What is a Sponsor? Sponsors make tax-deductible donations to CAI, a 501(c)(3), to enable CAI to continue its mission of igniting entrepreneurship, strengthening the economy, and maximizing the potential of promising inventions.

    Saving and Editing your CDA: You must answer all of the required questions (marked by an asterisk*) before submitting and saving your CDA. Once submitted, your form will be saved, and JotForm will send you an email confirming the submission along with an edit link for you to return to your form.

  • I certify that I have no actual or potential conflict of interest regarding teams I am leading, judging, mentoring, or advising. Examples of conflict of interest include, but are not limited to, direct personal/familial or financial relationship in connection with any of the business plans and live pitches to be reviewed or any of the individuals and entities that make up the teams.
