This is the Confidentiality Disclosure Agreement Form (CDA) for Phase 0 of the 2021-2022 Innovate Children’s Health Challenge. Each member of your team, including the Team Leader, should fill out and submit this CDA form.
All team member CDAs must be submitted before your team can be accepted into the contest. Additional members can be added throughout the contest via this form.
Saving and Editing your CDA: You must answer all of the required questions (marked by and asterisk*) before submitting and saving your CDA. Once submitted, your form will be saved, and JotForm will send you an email confirming the submission along with an edit link for you to return to your form.

Team Member Definitions:
Core Team Member: a person who is accountable for deliverables
Mentor: a person who has domain experience and will actively work with the team on a daily basis as the team needs
Advisor: a person with domain expertise and provides Q&A sessions for teams
Collaborator: an entity that can improve a team's probability of success; includes seed funders and services firms, e.g., Clinical Research Outsourcers, law firms, etc.
Team Structure:
Disciplines: Teams must be cross-functional, representing business, medical, legal and entrepreneurial disciplines. It also may be useful for teams to have computer science and/or engineering expertise, depending on selected inventions
Background: Teams may consist of members from various backgrounds and organizations
Team Size: As large as the Team Leader decides; minimum of 2
Age: No restrictions on age; participants under the age of 18 are required to sign a waiver
Recommended Team Member: Seasoned entrepreneur, who has founded a Life Sciences, biomedical and/or health IT company; raised dilutive and non-dilutive capital for that company and have exited either successfully or unsuccessfully. We prefer entrepreneurs that have tenure of at least five years (a minimum of three years) in a start-up and also have had corporate experience
We also recommend that you include collaborators, mentors and advisors on your team; however, they are not mandatory
