  • Please ensure that all the requested information is filled out for the Membership Dues to automatically calculate the applicable taxes and display the membership dues payable at the end of the form. 

  • Note: Private contact info is for CAND office use only and will not be made available to any other party in accordance with our privacy policy. As a Professional Association, it is incumbent upon us to collect this information from our members for security and audit purposes.

    To that end, we require that you provide your personal email as we will not accept an educational (.edu) or general admin clinic email. This personal email is used to contact you directly and becomes part of your private and secure login credentials for your CAND.ca member portal and professional profile.

    Your personal information entered above, other than your name, will NEVER be made public by/through the CAND unless you decide to make it part of your public facing profile.


    Important: CAND membership is linked with the provincial and territorial associations. Completing and submitting this form with your payment details below will begin the membership activation process. 

    After payment is finailzed by the CAND accounting department, you will receive your formal acceptance by email from the CAND Registrar, with your CAND.ca membership login credentials.

    New Ontario registrants must upload a copy of CONO’s “Step3” Eligibility form.

  • NOTE: Associate Members are non-practicing, on maternity leave, out-of-country, retired or on sabbatical and maintain coverage by and participation in all CAND member benefits but are not listed on the CAND.ca FindYourND search engine.

    Membership is based on the calendar year (January 1st to December 31st).

    • If joining in January, dues will be charged for the full year
    • If joining from February 1 to August 31, member dues will be pro-rated for the balance of the calendar year (to December 31st).
    • If joining after August 31st, members will be charged pro-rated dues for the balance of that calendar year as well as member dues for the following year.

     

  • Canadian Members: Applicable GST/HST taxes, based on your provincial or territorial place of residence, will be automatically calculated and applied at check-out as follows:

           AB/BC/SK/MB/NT/NU/YT: 5%               ON:13%                QC:14.975%            NB/NL/NS/PE: 15%

     
  • CAND POLICIES:

    Membership

    Membership is due January 1st and is expected within 30 days of that renewal date, otherwise the membership will lapse. If this occurs, the CAND must advise the relevant provincial/territorial association and Paisley-Partners Inc. that the member is no longer in good standing with the CAND.

    Lapse in membership will result in the cancellation of all membership benefits including any malpractice insurance through Paisley-Partners Inc.

    Membership will not be active until all applicable membership dues have been received and processed by the CAND.

    Change of Membership Category

    Any member requesting a change in membership category must inform the CAND in writing (by letter or email) at least one month in advance. The notice must include the date the change is to come into effect and the membership status requested. If the member will not be in practice (i.e. maternity leave, sabbatical, etc), they must maintain Associate Member status in order to continue to receive member benefits including malpractice insurance through Paisley-Partners Inc.

    Membership Cancellation

    Members requesting cancellation of their membership are required to inform the CAND of their request in writing (by letter or email) at least one month in advance of the requested cancellation date. Any refund due to cancellation of membership will be processed 30 days from the date of the written submission.

    When canceling their membership, members who are insured through Paisley-Partners Inc. are advised that their insurance coverage will be cancelled as well as any other members benefits received through other affiliate companies. An administration fee will be charged for membership cancellation and any subsequent membership reinstatement during the calendar year.

    Privacy Policy

    The CAND collects personal information for contact purposes only and may share said info with it's provincial constituent associations upon request. The CAND does not sell said membership information and/or mailing list to any third party for commercial purposes. Clinic information is provided to those companies supplying member benefits.
