Art Festival Liability Waiver

I, First Name Last Name , hereby agree to participate in the Evergreen Elk and Art Festival organized by Colorado Creative Music, Inc. dba ColoradoArtWeekend.com and ColoradoArtShows.com, scheduled to take place from 12:01 am Friday Sep 27th to 11:59pm on Sunday September 29th at 3719 Evergreen Parkway * Evergreen CO * 80439.

Any reference to "the art festival" is a reference to the Evergreen Elk and Art Festival.

As a requirement for being allowed to participate in this event, I acknowledge and agree to the following:

Assumption of Risk: I acknowledge that participation in the Evergreen Elk and Art Festival involves certain risks, including but not limited to, physical injury, property damage, loss of property due to theft, loss of property due to weather, and other dangers associated with public events.

Release of Liability: I hereby release and discharge coloradoartweekend.com, coloradoartshows.com, their respective affiliates, sponsors, partners, employees, volunteers, and representatives (hereinafter collectively referred to as the "Released Parties") from any and all claims, demands, causes of action, damages, or liabilities whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, while participating in the art festival.

Indemnification: I agree to indemnify and hold harmless the Released Parties from and against any and all liabilities, claims, actions, damages, costs, or expenses (including reasonable attorney's fees) arising out of or related to my participation in the art festival.

Medical Treatment: In the event of any injury or illness that I may sustain during the Evergreen Elk and Art Festival, I authorize the event organizers to seek and consent to any necessary medical treatment on my behalf. I understand that I will be responsible for any medical expenses incurred.

Photography and Publicity: I grant the irrevocable right to use my name, likeness, voice, and any photographs, video recordings, or other media taken of me during the Evergreen Elk and Art Festival for promotional purposes without compensation.

Compliance with Rules and Regulations: I agree to comply with all rules, regulations, and instructions provided by the event organizers and to behave responsibly and safely during the Evergreen Elk and Art Festival.

Severability: If any provision of this waiver is found to be unenforceable or invalid, the remaining provisions shall remain in full force and effect.

By signing below, I acknowledge that I have read this waiver and fully understand its contents. I voluntarily agree to be bound by its terms and conditions.
