I give permission for my name, likeness and images, captured on behalf of Central Dakota Children's Choir through video, photo and digital camera to be used for Central Dakota Children’s Choir publications and media. I understand these may be used in print publications, online publications, presentations, websites and social media. I also understand that no royalty, fee or other compensation shall be come payable to me by reason of such use. I hereby release and discharge Central Dakota Children's Choir from any and all claims and demands arising out of or in connection with Central Dakota Children’s Choir’s exercise of the rights granted herein.

I recognize and acknowledge that there are certain risks of illness/injury to participate in this program, and I agree to assume the full risk of any such injuries, damages, or loss regardless of severity which I may sustain as a result of participating in any activities connected or associated with this activity. I waive and relinquish all claims that I or my insurer may have against the Central Dakota Children’s Choir and its officers, agents, servants, and employees from any and all claims from injuries, damages, or loss which I may have or which may accrue to me on account of my participation in this program.
