In this form, we collect personal information for the purposes of the administration of counseling services to small businesses operating or intending to operate in the business Starter Company Plus/Start-Up Company program under Article 108 of the Act Municipal, 2001, sO 2001, C. 25. This information may also be used to contact you in connection with other initiatives, programs and services that the service Centre provides small business from time to time. TAX TREATMENT OF GRANTS

Grants received from Start-up Company are taxable under the laws of income tax of Canada and Ontario. T4A a receipt for income tax purposes will be issued to the recipient of the grant.

Commitments & Considerations   :
Use of Funds and Carry out the Program:
Any misuse of the grant will be converted immediately into a repayable loan.
All program requirements must be met in order to receive the final disbursement of the grant; the Eligible Participant will do, this includes, but is not limited to the following:

• Confirmation of participation in training programs and meetings with mentor;
• Acquire individual Learning Plans for the Program;
• Ensure the following terms and conditions are included in the Starter Company Plus Grant Agreement as prepared by the Recipient:
                               • Learning Plan: The Eligible Participant agrees to undertake the Learning Plan as a condition of their participation in the Program;
                               • Post-Program Progress Reporting: The Eligible Participant shall, if requested by the Recipient of Her Majesty the Queen in right of Ontario as represented by the Minister of Economic Development and Growth, provide progress reporting information such as the Eligible Participant’s business expansion and job creation for three consecutive Funding Years following the expiry date or termination of the Starter Company Plus Grant Agreement.
• Fill out a report of all the objectives achieved;
• Be able to provide the documents requested and received.

For the purposes hereof, "I", "me", "you", "your" refer to the undersigned or the undersigned on his own behalf or on behalf of each of the companies identified on this form.
"PREC" will be used in replacement of Prescott and Russell Entrepreneurship Centre;

"Personal Information" means personal, financial and other information about you that you provide to PREC and the data obtained from other sources outside the PREC, including to consultants and professional advisers.

PREC is required to protect your personal information in accordance with the requirements of the Act in this regard. PREC may collect personal information about you from time to time, including those obtained at the date hereof or before or after it, in order to improve the service offer that is made to you. PREC may provide your personal information to its donors for the purpose of making statistics and audit reports, as well as professional advisors and consultants. PREC does not sell or does not disclose your personal information to third parties except as described herein or if required by law. ELIGIBILITY

I hereby agree that, in order to apply to participate in the Starter Company Plus Program, I am meeting the following Program eligibility requirements:
·          18 years of age or older at the time of application;
·          Proposing a new company, expansion of an existing company or buying a business in Ontario;
·          Not attending school full time nor returning to school;
·          Resident of Ontario;
·          Canadian citizen or permanent resident;
·          Not enrolled in other provincial employment or self-employment related programs that include or do not include financial assistance;
·          Not currently enrolled concurrently in any self-employment or entrepreneurship training/financing programs offered by government-funded organizations.

·Not have previously received a grant under the Starter Company Plus Program.

CERTIFICATION I hereby certify that the statements and information I gave in this application is true and accurate to my knowledge, and I authorize the Small Business Enterprise Center to investigate all statements and information contained in this form and in any documents attached to it, unless I have made a written declaration to the contrary. I understand and acknowledge that any misrepresentation, falsification or omission of information of consequence in this application may result in rejection of my application under the Starter Company Program.
