• CFA Fiscal Sponsorship: Grant Information Request Form

  • This form is for fiscally sponsored CFA projects requesting organizational documents or information needed to complete grant applications. Please complete sections in detail and upload any grant materials provided by the funder. CFA requires at least 5 business days’ notice to prepare and return requested documents (not including notarized documents or certified copies). Incomplete requests or those submitted with less than 5 business days’ lead time will not be fulfilled on time.

  • If a grant or funder requires a notarized or certified copy of a CFA document, CFA can facilitate this service. This may include notary signatures, certified copies, and mailing fees. A minimum fee of $65 applies.

    For general notarization needs, CFA offers an in-house notary service. To use this service, please email us to schedule an appointment. Notarizations must be completed in person at the CFA Studio in Los Feliz, California. Please bring the document(s) along with two forms of identification, including one government-issued photo ID.

     
    Fees: $15 per signature plus a $65 convenience fee (minimum). Additional charges may apply for multiple signatures or certified copies.
