  • I AGREE THAT ALL STATEMENTS I HAVE MADE ON THIS FORM ARE TRUE. IF IT IS FOUND THAT ANY STATEMENTS I HAVE MADE ON THIS FORM ARE NOT TRUE, THE ADOPTED ANIMAL CAN BE CONFISCATED. I HAVE READ AND FULLY UNDERSTAND THE TERMS AND CONDITIONS OF THIS AGREEMENT, AND AGREE THAT IT IS NOT ARBITRARY AND THAT THE PROVISIONS HEREIN ARE COMMON IN A PET ADOPTION AGREEMENTS. I UNDERSTAND THAT IF I FAIL TO COMPLY WITH ANY OF THE TERMS SPECIFIED HEREIN, COCO'S HEART DOG RESCUE HAS THE RIGHT TO RECLAIM THE DOG AND PURSUE ANY LEGAL CHANNELS TO DO SO AT MY EXPENSE. (COURT FEES, ATTORNEY, PARALEGAL FEES AND ADMIN FEES).

  • At Coco's Heart, we value high quality veterinary care. That is why we perform a variety of healthscreening tests on incoming dogs which may include:

    • Heartworm test (age dependant)
    • Lyme/Ehrlichia/Anaplasmosis tests
    • Fecal tests
    • Blood panel tests
    • Complete Blood Counts
    • Brucellosis screening (source dependant)

    While we do the best we can to screen for any potential contagious diseases, it is possible for some diseases to go undetected. This can happen for a variety of reasons and does NOT mean a test was inappropriately administered. There is a known risk of "false negative" testing. Without concurrent clinical symptoms that may correlate to a specific disease, Coco's Heart does not routinely re-test negative results. Adopters will need to discuss prudent re-testing with their own veterinarian based on each individual animal.

  • Obedience Course Requirement Addendum

    The benefits of obedience classes go far beyond basic commands. The socializing, bonding and basic skills young dogs learn in these settings is priceless. We are hoping to set our dogs and adopters up for success, regardless if a member of the family has already attended a class in the past. If you need resources for trainers, please let us know!
  • Trial Addendum

    CHDR offers a one-week (7 days) trial for dogs aged one year and older. If you are given a one-week trial, you must reach out during the 7 day period if the dog is deemed not a fit for your home. After the week trial, the adoption is considered finalized and a formal surrender process must be followed. Please note- The trial run is only offered for dogs one year old and older.

    Coco's Heart values a life-long commitment to each of our dogs. We have been able to uphold that commitment to all of our dogs in most cases, however in cases of aggression or reactivity, our options in rehoming or placing in our foster program again are limited. For that reason, we want to provide clear information at the time of adoption so that adopters can be prepared for what to expect during the return process- regardless of if shortly after adoption, or several years after adoption.

    Timeline

    Coco's Heart strives to welcome all dogs back to rescue within 2 weeks once a Surrender Inquiry is received. The Surrender Inquiry is therefore the time-stamp that provides us a 2-week notice. While our team works as quickly as we can, that 2-weeks is important to ensure we can move through our internal processes with returned dogs. If a dog is adopted and a Surrender Inquiry is received 3 days later, we still may need that full 2 weeks, regardless of the fact that it was shortly after adoption.

    Aggression/bite history

    Coco's Heart values our dogs, our adopters, and our community. We evaluate each situation individually, but in general cannot place dogs that are determined to be aggressive or have bite histories. What does this mean? This means that Coco's Heart may determine that they cannot welcome the dog back to rescue and/or recommend euthanasia. It is very sad, and we hope it does not come to that, but we want to be clear that histories of aggression need to be taken very seriously. If we cannot welcome a dog back to rescue, then we encourage adopters to pursue other options such as euthanasia instead of pursuing the surrender process. In those hard moments it is comforting for the dogs to be with their family, and not with strangers to them.
