I understand that Discovery Camp is organized by Champion Life Centre (Guelph)'s Children's Ministry, the Champion KIDS. As such, I consent to my child(ren)'s learning of Christian Bible stories, biblical truths, and Christian spiritual formation with the understanding that all counselors, group leaders, and/or facilitators of Discovery Camp have completed all necessary training and background checks.
I understand that Champion Kids Guelph, a ministry of Champion Life Centre (Guelph) agrees to manage any personal identifiable information related to my child(ren) in accordance with the the church's privacy policy.
I, the Parent/Guardian of the Discovery Camp participant(s) named in this registration, release and forever discharge and hold harmless Champion Life Centre and their respective officers, employees, volunteers, directors, agents, affiliates, successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from his/her/their participation in the Discovery Camp of Champion Life Centre (Guelph). I understand and acknowledge that this Waiver and Release discharges Champion Life Centre from any liability or claim that I, the Parent/Guardian of the Discovery Camp participant(s) named in this registration may have against Champion Life Centre with respect to bodily injury, personal injury, illness, death or property damage that may result from my activities and time spent at the Discovery Camp. I also understand that Champion Life Centre does not assume any responsibility for or obligation to provide financial assistance or other assistance to me and to my child(ren), including but not limited to medical, health or disability insurance, in the event of injury, illness, death or property damage.
I, the Parent/Guardian of the Discovery Camp participant(s) mentioned in this registration understand that I expressly waive any such claim for compensation or liability on the part of Champion Life Centre beyond what may be offered voluntarily and freely by a representative of Champion LIfe Centre, in its sole discretion, in the event of such injury or medical expense.
I hereby release and forever discharge Champion Life Centre from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered to my child(ren) in connection with an emergency during his/her/their time with Champion Life Centre (Guelph)'s Discovery Camp.
I understand that my child(ren)'s time with Champion Life Centre (Guelph)'s Discovery Camp may include activities that may be hazardous to him/her/them, including, but not limited to physical activities, such as but not limited to, swimming, games, running, and other sports-related activities, as well as local transportation to and from the camp site. I hereby expressly and specifically assume the risk of injury or harm in these activities and release Champion Life Centre from all liability or injury, illness, death, or property damage resulting from my child(ren)'s participation in these activities and his/her/their time at the Discovery Camp.
I, the Paren/Guardian of the Discovery Camp participant(s), understand that the camp organizers, staff, and volunteers will make every effort to maintain a safe and positive environment for all campers. However, in the event that my child(ren) engages in behaviour that poses a risk to the safety and well-being of themselves, other campers, or volunteers, I agree to the following:
1. Immediate Removal: the camp organizers reserve the right to immediately remove the Discovery Camp participant(s) from the camp premises if, in their judgement, my child's behaviour jeopardizes the safety or well-being of themselves, other campers, or volunteers.
2. Rescinding Registration: I acknowledge and agree that if my child(ren) engages in behaviour that causes harm or disruption to other campers or volunteers, the camp organizers may, at their discretion, rescind my child(ren)'s registration from the camp without refund.
3. Parent/Guardian Notification: In the event of my child(ren)'s removal from the camp premises due to behavioral and/or safety concerns, I agree to be promptly notified by camp staff or organizers. I understand that it is my responsibility to arrange for my child's immediate pickup from the camp location.
4. Release of Liability: I hereby release and hold harmless Champion Life Centre (Guelph), its officers, directors, employees, and volunteers from any and all claims, damages, or liabilities arising out of my child's participation in the camp, including but not limited to incidents involving my child's behavior.
I have read the entire Release, Waiver of Liability, Indemnity Agreement and Parental/Guardian Consent Agreement and understand that I am signing it freely and voluntarily permit my child(ren) to participate in the Discovery Camp.
