In consideration for consenting to my child(ren)’s participation at the Change Conference 2026, I hereby freely and voluntarily, without duress, execute this Agreement under the following terms:

1) Waiver and Release. I, the Parent/Guardian of minor participant(s)/visitor(s), release and forever discharge and hold harmless Champion Life Centre and their respective officers, employees, volunteers, directors, agents, affiliates, successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from their participation in the above-stated event/activity approved by Champion Life Centre (Guelph). I understand and acknowledge that this Waiver and Release discharges Champion Life Centre (Guelph) from any liability or claim that the participant/volunteer/visitor, and/or Parent/Guardian of minor participant/visitor/volunteer may have against Champion Life Centre (Guelph) with respect to bodily injury, personal injury, illness, death or property damage that may result from my child(ren)'s activities and time spent at the above-stated event/activity. I also understand that Champion Life Centre (Guelph) does not assume any responsibility or obligation to provide financial assistance or other assistance to me or my child(ren), including but not limited to medical, health or disability insurance, at the event of injury, illness, death or property damage as a result of participating in the above-stated event/activity.

2) Insurance. I, the Parent, Guardian of minor participant/visitor/volunteer, understand that I expressly waive any such claim for compensation or liability on the part of Champion Life Centre (Guelph) beyond what may be offered voluntarily and freely by a representative of Champion Life Centre (Guelph), in its sole discretion, in the event of such injury or medical expense.

3) Medical Treatment. I hereby release and forever discharge Champion Life Centre from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered to me or my child(ren) in connection with an emergency during my child(ren)'s time at the above-stated event/activity.

4) Assumption of Risk and Indemnification. I understand that my child(ren)'s time at the above-stated event/activity may include activities that may be risky or hazardous to them, including, but not limited to, loading and unloading of heavy equipment and materials, other physical activities, such as but not limited to games, running, and other sports-related activities, as well as any local transportation to and from the venue. I hereby expressly and specifically assume the risk of injury or harm in these activities and release Champion Life Centre (Guelph) from all liability or injury, illness, death, or property damage resulting from my child(ren)'s participation in these activities during the above-stated event/activity.

I,   Parent's First Name   Parent's Last Name   agree for my child(ren) to be transported to/from the above pick-up/drop-off address by a CLC-approved volunteer carpool driver.

I am accepting full responsibility and liability for my child(ren). I agree on behalf of myself, my heirs, successors, and assigns, to hold harmless and fully release Champion Life Centre (Guelph), its staff, directors, affiliate churches, and volunteers, from any claim, demand or cause of action arising from or in connection with the transportation to and/or from their activity. I understand that transportation being provided for my child(ren) is completely voluntary and that I am totally responsible for my child(ren) and release Champion Life Centre (Guelph), its staff, directors, affiliate churches, and volunteers and the designated volunteer drivers from any liability.
