I Parent/Guardian Full Name*, have agreed to allow my child(ren), Child(ren)'s Full Name(s)* to be transported to and/or from Champion Life Centre Guelph's Discovery Camp located at 55 Devere Dr, Guelph ON, by designated volunteer drivers of Champion Life Guelph. I am accepting responsibility and liability for my child(ren). I agree on behalf of myself, my heirs, successors, and assigns, to hold harmless and release Champion Life Centre (Guelph), its staff, directors, affiliate churches, and volunteers, from any claim, demand or cause of action arising from or in connection with the transportation to and/or from their activity. I understand that transportation being provided for my child(ren) is completely voluntary and that I am totally responsible for my child(ren) and release Champion Life Centre (Guelph), its staff, directors, affiliate churches, and volunteers and the designated volunteer drivers from any liability.
